What to Bring to Your Appointment

  • Your insurance card
  • Your migraine Log (atleast 7 to 14 days)
  • A list of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent x-rays, cat scans, MRI’s or appropriate records
  • You may expedite the new patient process by reviewing and signing the downloadable forms for New patients.

 

Payment and Insurance Information

We understand that insurance benefits can be confusing and unclear to most patients.  It is always best to call us for any clarifications or review your benefits with your carrier.

Most insurance carriers will pay for all or part of  your visit at the Migraine Relief Center. We generally do not bill your insurance, with a few exceptions. We provide instructions on how insurance billing claims should be submitted.  Anything not covered by insurance will be your responsibility.  Financial Counselors will review your benefits and are available to assist you.

We have reasonable consultation fees, and also have plans available for multiple visits.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.
